Hypothyroidism: Goitre and Cretinism

Biology · Chemical Coordination and Integration · NEET

Hypothyroidism means the thyroid gland makes too little thyroid hormone (T3 and T4). The most common cause is iodine deficiency in the diet, which makes the thyroid gland enlarge and swell in the neck. This visible swelling is called goitre. If hypothyroidism happens in a pregnant mother, the baby's growth and brain development are harmed, causing a condition called cretinism (stunted growth and low IQ). Memory hook: iodine LOW to grow, so the gland swells to GO-itre, and the baby stays small like a CRE-tin.

Iodine deficiency lowers T3/T4 (hypothyroidism). The pituitary releases more TSH, so the thyroid enlarges into a goitre. If it happens during pregnancy, the baby develops cretinism with stunted growth and low IQ.

Your doubts, answered

Is goitre caused by low or high thyroid hormone? I keep getting confused.

Simple goitre (the one NCERT links to iodine) is caused by LOW thyroid hormone, i.e. hypothyroidism. When iodine is missing, the thyroid cannot make enough T3 and T4, so the pituitary keeps sending TSH to push it harder. The over-stimulated gland grows bigger and bigger, giving the neck swelling called goitre. So for the iodine-deficiency case, remember: low hormone, big gland.

Then why does my textbook also say goitre in Graves' disease (hyperthyroidism)?

Because the word goitre only means an enlarged thyroid gland, not a single disease. It can appear in BOTH directions. In iodine-deficiency hypothyroidism the gland enlarges from too little hormone; in Graves' disease (hyperthyroidism) the gland enlarges from over-activity and the eyeballs bulge out (exophthalmic goitre). NEET tests this: 'goitre' alone is not enough, read the cause. This page is about the hypothyroid/iodine-deficiency goitre.

What exactly is cretinism and how is it different from goitre?

Cretinism is the childhood/infant result of hypothyroidism, usually when the mother had low thyroid hormone during pregnancy. The baby shows stunted growth (dwarf-like body), low intelligence quotient, abnormal skin and deaf-mutism. Goitre is a neck swelling in a person of any age; cretinism is defective growth and mental development in a developing baby. Same root cause (low thyroid hormone), different visible result.

Does iodine deficiency cause hypothyroidism or hyperthyroidism?

Iodine deficiency causes HYPOthyroidism. Iodine is a raw material the thyroid needs to build T3 and T4. No iodine means the gland cannot make enough hormone, so hormone levels fall. This is why iodised salt is used to prevent goitre. Do not confuse this with hyperthyroidism (Graves' disease), which is over-production, not a deficiency.

What happens to hypothyroidism in adult women?

NCERT specifically notes that in adult women, hypothyroidism may make the menstrual cycle become irregular. This is a favourite one-line NEET fact. So the effects of low thyroid hormone are: goitre (iodine deficiency), cretinism (in the growing baby), and irregular menstrual cycle (in adult women).

⚠️ The NEET trap
Ticking 'goitre' as a sign of hyperthyroidism only, or matching Thyroxin with Grave's disease.
Goitre (simple, iodine-deficiency type) belongs to HYPOthyroidism / low thyroxin; Grave's disease (exophthalmic goitre) is the hyperthyroid one. In match-the-column, Thyroxin pairs with Goitre.
🧠 Goitre can sit on BOTH sides of the fence. When the question says iodine deficiency or just 'goitre', pick low thyroid hormone; when it says protruding eyeballs, pick hyperthyroidism.

Real NEET questions

2024

Match List I with List II. List I A. Exophthalmic goiter B. Acromegaly C. Cushing's syndrome D. Cretinism List II I. Excess secretion of cortisol, moon face & hyperglycemia II. Hypo-secretion of thyroid hormone and stunted growth III. Hyper secretion of thyroid hormone & protruding eye balls IV. Excessive secretion of growth hormone Choose the correct answer from the options given below

A · A-IV, B-II, C-I, D-III
B · A-III, B-IV, C-II, D-I
C · A-III, B-IV, C-I, D-II
D · A-I, B-III, C-II, D-IV
Solution: Cretinism (D) is due to hypo-secretion of thyroid hormone with stunted growth, so D-II. Exophthalmic goitre (A) is hyperthyroidism with protruding eyeballs, so A-III. Acromegaly (B) is excess growth hormone (IV) and Cushing's syndrome (C) is excess cortisol with moon face (I). This gives A-III, B-IV, C-I, D-II = option C.
2019

Match the following hormones with the respective disease. Column I (a) Insulin (b) Thyroxin (c) Corticoids (d) Growth Hormone Column II (i) Addison's disease (ii) Diabetes insipidus (iii) Acromegaly (iv) Goitre (v) Diabetes mellitus Select the correct option

A · (a) (v), (b) (i), (c) (ii), (d) (iii)
B · (a) (ii), (b) (iv), (c) (iii), (d) (i)
C · (a) (v), (b) (iv), (c) (i), (d) (iii)
D · (a) (ii), (b) (iv), (c) (i), (d) (iii)
Solution: Thyroxin deficiency (hypothyroidism from iodine deficiency) enlarges the thyroid, so Thyroxin pairs with Goitre (iv). Insulin lack causes Diabetes mellitus (v), Corticoids under-production causes Addison's disease (i), and excess Growth Hormone in adults causes Acromegaly (iii). This gives a-v, b-iv, c-i, d-iii = option C.

Frequently asked

Is goitre a symptom of hypothyroidism or hyperthyroidism?

Goitre simply means an enlarged thyroid gland and can occur in both. The iodine-deficiency simple goitre that NCERT stresses is a sign of hypothyroidism (low thyroid hormone). Exophthalmic goitre with bulging eyes is a sign of hyperthyroidism (Graves' disease).

What causes cretinism?

Cretinism is caused by hypothyroidism during pregnancy. Low thyroid hormone impairs the growth and brain development of the baby, leading to stunted growth, low intelligence quotient, abnormal skin and deaf-mutism.

Why does iodine deficiency cause the thyroid to enlarge?

Iodine is needed to make T3 and T4. Without iodine, hormone output falls, so the pituitary keeps releasing TSH to stimulate the gland. This continuous stimulation makes the thyroid grow larger, producing the neck swelling called goitre.

How is cretinism different from dwarfism caused by growth hormone?

Cretinism is due to low thyroid hormone and includes low IQ, abnormal skin and deaf-mutism along with stunted growth. Pituitary dwarfism is due to a lack of growth hormone and the child usually has normal intelligence and body proportions. NEET often tests this contrast.

What effect does hypothyroidism have in adult women?

NCERT notes that in adult women hypothyroidism may make the menstrual cycle become irregular. This is a commonly asked one-line fact.
