Simple goitre (the one NCERT links to iodine) is caused by LOW thyroid hormone, i.e. hypothyroidism. When iodine is missing, the thyroid cannot make enough T3 and T4, so the pituitary keeps sending TSH to push it harder. The over-stimulated gland grows bigger and bigger, giving the neck swelling called goitre. So for the iodine-deficiency case, remember: low hormone, big gland.
Because the word goitre only means an enlarged thyroid gland, not a single disease. It can appear in BOTH directions. In iodine-deficiency hypothyroidism the gland enlarges from too little hormone; in Graves' disease (hyperthyroidism) the gland enlarges from over-activity and the eyeballs bulge out (exophthalmic goitre). Cretinism is the childhood/infant result of hypothyroidism, usually when the mother had low thyroid hormone during pregnancy. The baby shows stunted growth (dwarf-like body), low intelligence quotient, abnormal skin and deaf-mutism. Goitre is a neck swelling in a person of any age; cretinism is defective growth and mental development in a developing baby. Same root cause (low thyroid hormone), different visible result.
Iodine deficiency causes HYPOthyroidism. Iodine is a raw material the thyroid needs to build T3 and T4. No iodine means the gland cannot make enough hormone, so hormone levels fall. This is why iodised salt is used to prevent goitre. Do not confuse this with hyperthyroidism (Graves' disease), which is over-production, not a deficiency.
NCERT specifically notes that in adult women, hypothyroidism may make the menstrual cycle become irregular. This is a favourite one-line NEET fact. So the effects of low thyroid hormone are: goitre (iodine deficiency), cretinism (in the growing baby), and irregular menstrual cycle (in adult women).
