What is the difference between humoral and cell-mediated immunity?

Cell-mediated immunity (CMI) uses T-lymphocytes that act directly, without secreting antibodies. So the simple split is: humoral = B-cells + antibodies in fluid; cell-mediated = T-cells doing the action.
Why is it called 'humoral' immunity?
NCERT says the antibodies are found in the blood, so the response is called the humoral immune response. The old word 'humor' meant body fluid. Because antibodies work while dissolved in blood and body fluids, this defence got the name humoral. This is a common NEET memory point: humoral = fluid/blood = antibodies.
Which cells are involved in humoral immunity and which in cell-mediated immunity?
Humoral immunity is carried out by B-lymphocytes, which produce antibodies. Cell-mediated immunity (CMI) is carried out by T-lymphocytes. Note one NCERT trap: T-cells do NOT secrete antibodies themselves, but they HELP B-cells to produce antibodies. So T-cells assist humoral immunity but they mediate CMI directly.
Why is graft (organ transplant) rejection cell-mediated and not humoral?
When an organ is transplanted, the body sees the graft tissue as 'nonself'. NCERT states the cell-mediated immune response is responsible for graft rejection, because T-lymphocytes directly attack the foreign transplanted cells. Antibodies (humoral) act mainly against free-floating antigens in blood, not against solid transplanted tissue. This exact idea has been asked in NEET 2017 and NEET 2019.
Do T-cells make antibodies?
No. This is a frequent confusion. NCERT clearly says T-cells themselves do not secrete antibodies; only B-lymphocytes produce antibodies. T-cells only help B-cells make antibodies. So if a NEET option says 'T-cells secrete antibodies', it is wrong.
